At Silver Springs we believe that our Care and Support Plans are what set us apart from the others. When we receive a referal we move mountains to get to know you. Prior to your admission, we will carry out a full assessment of your specific healthcare, social and personal needs. We will look at your aspirations and short, medium and long term goals. We will capture your likes, dislikes and communication preferences to really get to know you. We will also assess any possible risks and work out how these risks may be managed.
We will invite you to spend time at Silver Springs, get to know us and feel comfortable. We encourage prospective service users to stay for meals, overnight or for the weekend. You will be have access to a Key Worker who will be responsible, under the supervision of the Manager, for agreeing the Support Package with you and your Social Worker. They will keep this under regular review and as your needs and preferences change, so will your Care Package.
Working in Collaboration
Silver Springs is supported by a GP Surgery which is located within two minutes walking distance from the Home. We will work closely with your GP as your Health and Wellbeing is our priority. Where required, we will request referrals to the local secondary services such as Physiotherapy, Psychology, Speech and Language, Orthotics, Chiropody etc. We also work closely with the Epilepsy Society.
We work in partnership with your Social Worker and Placing Authority who will review your care annually or sooner if required. We can also facilitate multi-disciplinary professionals meetings where we feel we need a more collaborative approach is required.
There are several Opticians and Dentists in the town and staff are always happy to assist with making appointments for any of these services. Hairdresser visits can be arranged if required, however, we are more than happy to support you to go to one of the many hairdressers in town.
Personal Care and other Practical Support
We can offer support with all aspects of your personal care depending of your level of needs. Below is a list of the areas of support we can provide:
- Oral care
- Continence care
- Assistance with administration and safekeeping of medication
- Assistance with getting up or going to bed
- Moving and positioning
We can also offer support with Practical Tasks such as:
- Making or changing beds
- Personal Shopping
- Supporting with banking, benefits and other administration.
- Assistance with laundry including ironing
- Preparing meals and Menu Planning
- Support to attend regualr health checks
- Light domestic tasks
- Developing Independent Living Skills.
- Support with Emails and other communication.
The home offers a number of daytime interests both within and outside the house. We have an “in-house Activities Organiser” who will support you to design an individual activities plan. In the house we offer a range of activities including Fun Cooking, Arts & Crafts, Ball Games, Wii Console, Music Therapy, Movie Sessions, Reading and Puzzles, Youtube Sessions, Gardening, Pampering and Relaxation, Sensory Activities and Independent Living Skills. We also support out service users to access a range of activities in the community such as going to the local Shops, Swimming Pool, Cinema, Bowling alley, Park, Petting Zoo, Theatre, Garden Centre. Watford has a wide range of other amenities within walking distance and London is only a train ride a way.
All religious needs are catered for at Silver Springs. This includes worship, diet and cultural customs or behaviours. There are C of E and Methodist Churches, Temples, Sikh Gurdwaras, a Synagogue and Mosque all nearby which you will be supported to attend if you wish.
Family, Friends and Visitors
Family and friends are welcome to visit at anytime. Just give us a call to make sure they are in. Service users are, of course, free to go out at any time with family members and friends, including overnight stays if they wish to. For safety reasons they are requested to advise the staff on duty of their absence and of the approximate time of their return. We can work closely with your loved ones in providing your support if you would like us to do this, however, we will always respect your right to privacy and confidentiality.
Meals and Nutrition
Food and drink bring enormous pleasure to our lives. Eating and drinking well have an important part to play in the health and wellbeing of our service users. Simple changes to what we eat and how much we eat can contribute to a better quality of life, and enabling eating well should always be seen as one of the most positive things we can do as part of providing support and good care. We are well versed in the principles of good nutrition, weight management, swallowing and physical difficulties, encouraging eating well and monitoring nutritional status. Our Key-Workers work with our Service Users and collaborate with Dieticians and other Health Care Professionals to produce Menu Plans to meet their specific needs.
